HOW DO I GET A COPY OF THE IMMUNIZATION RECORD ?
Call our office (972) 473-9063 and allow two business days for completion.
HOW DO I GET A COPY OF MEDICAL RECORDS?
An independent company handles the transfer and copying of medical records. The fee for their services will be set and handled by them.
Our office can provide a chart summary and/or shot record for a nominal fee.
PRESCRIPTIONS AND REFILLS
Initiate the process by calling the pharmacy. Ask them to fax a refill request to our
office at (972) 473-9059.
DO YOU HANDLE SAME - DAY SICK VISITS ?
Yes we do. We offer same-day appointments for sick visits.
REFERRALS
Suppose your insurance plan requires a referral for specialist or non-emergent procedures and/or lab tests. In that case, you must allow us five business days to complete the appropriate form(s) before obtaining services.
What should I do if my child gets sick after hours?
Please contact our after-hours answering service for assistance at 972-473-9063, where a provider will call you back at the earliest possible. For emergencies, please call 911 or take your child to the nearest emergency room.
